Appointment Policies

• Required Forms: The intake form and service agreement must be completed at least 24 hours before your appointment so I can review your information and prepare. If forms are not received on time, the appointment may need to be rescheduled.

• Rescheduling: Each appointment includes one complimentary reschedule, if needed.

• Cancellations & Refunds:

  • Cancellations made at least 48 hours in advance are eligible for a full refund.

  • Cancellations made within 48 hours of the appointment, or after a reschedule has already been used, are eligible for a 50% refund.
